Thanks are due to all those who were willing to share their data with me. In many cases the data is unpublished, and the authors have not presented their data as being of publishable quality, but have permitted its use for the purpose of establishing reconstructions, where a certain quantity of anomalous data can be outweighed by the bulk of data from other sources.
